Dr. Janson received her doctoral degree
from the University of California San Francisco in 1983 and joined
the faculty as a Professor of Nursing in the Department of Community
Health Systems and Adjunct Professor in the Department of Medicine.
Her major academic activities include teaching in the doctoral
and masters programs, asthma research, campus advisory and editorial
committees, and consulting. Dr. Janson was a member of both the
1991 and 1997 NHLBI Expert Panels on Asthma and currently serves on
the third Expert Panel 2005-2006. These expert panels wrote the
National Guidelines for the Diagnosis and Management of Asthma that
became the national standard of care for asthma. Dr. Janson is a
certified nurse practitioner and pulmonary clinical specialist and
maintains an active role in the Chest Faculty Practice at UCSF. She
is a Fellow in the American Academy of Nursing.
My primary research contributions are in
the field of pulmonary disease, especially the perception of respiratory
symptoms, the clinical and biological markers of asthma management
outcomes, and the impact of patient education on asthma control,
quality of life and treatment adherence. My research program has
included studies of variables that are affected by respiratory
status, such as peak expiratory flow, spirometric pattern of lung
function, dose responsiveness to methacholine, depression, life
satisfaction, markers of asthma morbidity, symptoms associated
with asthma, and biological markers of airway inflammation. Recent
work focuses on patient behavior and delays in seeking acute treatment
and medication adherence. It also focuses on cueing patient-clinician
collaboration and communication through graphic and interpreted trend
data of peak expiratory flow and spirometry. The interventions I have
studied include the effect of specific patient feedback of physiologic
information, such as airway resistance and peak expiratory flow, and
especially the effect of patient educational interventions and
self-management training on clinical and biological outcomes of asthma.
My earlier work focused on the perception
and interpretaion of the sensation of dypnea. As a continuation
of that work, I studied asthma self-care behaviors, reasons patients
delay seeking treatment for acute asthma, and the predictors of
the outcomes of living with asthma. In a series of clinical trials
begun in 1994, I studied the interface of clinical variables and
markers of inflammation in induced sputum to monitor the effects
of asthma self-management educational interventions. The outcomes
of these trials show that individualized asthma self-management
training significantly improves adherence to anti-inflammatory
therapy and decreases markers of inflammation in sputum. These
studies continue to focus on ways to improve adherence to treatment
through teaching behavior change and providing specific individualized
feedback of pulmonary function and airway inflammation data.
